Use rubber gel ankle guards
I would put band aids over where it rubs and then wear thick socks and pull my breeches down as far as they would go. My boots stopped hurting me once I broke them in
i have the same problem and equifit gel bands are the best
Equifit makes gel bands for that! They work great
I have this same problem, I wear tall socks and put band aids wear it rubs for shows
I use duck tape
Dover and other tack stores sell gel wraps that I wear on my ankles and knees. My tall boots are completely pain free now!

Have you heard about compeed? They have a sort of a stick that you can rub on your heels to stop friction
