Lots of lunge lessons and no stirrups work will help. Also ensure that your saddle fits you. Not all saddles fit all riders so it may be your saddle pushing you out of balance!
Help fix your tendency to slip your legs back. By the way I used to have the same problem. Easy fix!
You have to grip with your claves a lot and make sure that you are bending your knee when you rise trot. In the canter think heels down legs forward and that usually helps. If not, contact your instructor and ask her/him for so tips, strategies or exercises to
Make sure you have your leg around him at all times. It may be hard to start off with but it becomes second nature. Practice makes perfect
Check how you're using your inner thigh. It may be that you point your toe out in irons which causes you to lose stability in your leg. A strong inner thigh usually corrects lower leg instability.

I know this sounds pricy and maybe out of the picture... but maybe consider a new saddle?
